Incident Report Form

CPAN'S Incident Report Team carefully reviews and triages each incident report based on urgency, relevance, and available resources. While every submission is important, response times may vary depending on the number of reports received and the complexity of the issue. CPAN is committed to connecting families with trusted legal, medical, media, and advocacy partners when appropriate, and to ensuring that every report contributes to greater accountability and transparency in Colorado’s institutions.

The Colorado Parent Advocacy Network (CPAN) is dedicated to supporting parents, students, educators, and concerned Coloradans who are standing up for truth, transparency, and accountability. If you have evidence of harm or discrimination, including on the basis of religion, race or sex, or concerns related to compelled speech, safety, controversial lessons, DEI initiatives, academic issues, special education, drugs, or mental/medical healthcare, we encourage you to submit your report here. Our scope includes reports involving schools, healthcare systems, family courts, and child protective services when those institutions overstep or violate a parent's authority to direct the upbringing, care, or education of their child.

A CPAN Advocate will follow up with you to ensure your concerns are heard and documented.

If CPAN is able to provide direct support for your case, we will ask you to sign a confidentiality agreement. This agreement protects both you and CPAN, ensuring that sensitive information and ongoing investigations remain secure and handled responsibly.

CPAN does not charge claimants for support or advocacy. Our ability to assist families, conduct investigations, and connect with media and legal partners is made possible entirely through the generosity of our donors who believe in our mission to protect children and defend parental rights.
